Diakon Lutheran Social Ministries is Hiring a Case Manager II (Intensive Therapeutic) Near Topton, PA
- Responsible for identifying and documenting the special service needs of clients (whether the services are readily available) and matching the client with appropriate services.
- Makes regular contact with clients as well as participates in group functions for children, biological, and foster families as required.
- Acts as a liaison with public and private sector workers.
- Keeps client, resource, fiscal, and statistical records as dictated by good practice and required by regulation.
- Childline & DHS FBI clearance results are required prior to beginning employment.
- Bi-lingual/Spanish preferred
