Diakon Lutheran Social Ministries is Hiring a Case Manager II Near Allentown, PA
- Responsible for providing social services to residents.
- Researches and documents new resident/client's social history and completes an admissions note; schedules conferences with residents' or clients' families as required.
- Provides direct services to residents and families including social assessment, casework services, and discharge planning.
