JOB SUMMARY

RESPONSIBILITIES :

The Home Care Representative facilitates referrals for Medicare Certified home care services to ensure appropriate continuity of patient care.

Acts as liaison between CareResource Hawaii and its primary source of referrals.

II. TYPICAL PHYSICAL DEMANDS :

• Sitting, finger dexterity, seeing, hearing, speaking.
• Constant : reaching above, at and below shoulders; repetitive arm / hand motion, and frequent gripping of an object.
• Frequent : standing, walking, stooping / bending, and twisting body.
• Infrequent : lifting, pushing, and carrying items up to 15 pounds.

IV. MINIMUM QUALIFICATIONS :

A. EDUCATION / CERTIFICATION AND LICENSURE :

• High school diploma or equivalent.
• Current Hawaii State license as a Registered Nurse or Physical Therapy.
• Valid U.S. driver’s license, vehicle and vehicle insurance as required by Hawaii State law.
• Driver’s abstract or evidence of good driving record.
• Current BLS certification.

B. EXPERIENCE :

• Two (2) years’ experience as a registered nurse or physical therapist in a clinical setting.
• Demonstrated proficiency in Microsoft Office, Word, and Excel.
• Experience to demonstrate :

o Skill in problem identification and resolution.

o Ability to establish and maintain effective working relationships with clients, staff, and the public.
