Crossroads is a leading addiction treatment provider of outpatient medication-assisted treatment (MAT). We treat patients with opioid use disorder (OUD) using medications such as methadone and suboxone / buprenorphine.

We pride ourselves in supporting our patients’ medical and personal recoveries from substance use disorder. Starting our fight against the opioid addiction crisis in 2005, Crossroads has remained physician led and patient focused as we’ve grown to 100 clinics across nine states.

Assist in developing and monitoring recovery plans in collaboration with patients

Teach, role model, and coach patients on how to access helpful resources within their community and healthcare systems, how to cultivate best practices for management of symptoms related to their treatment, and assist with developing necessary skills to attain and maintain employment / housing.

Work with patients to identify how they are reacting to prescribed treatment medications and communicate any impactful changes with any necessary medical providers related to the patients treatment.

At minimum, High School Diploma or GED required.

  • Must be at least eighteen (18) years of age or older; have current or past mental illness, substance abuse, or co-occurring disorder;
  • self-identify as a person who is in recovery from mental illness, substance abuse, or co-occurring disorder as part of his or her personal recovery process;

have demonstrated self-directed recovery for a minimum of 24 consecutive months.

Experience in healthcare, addiction medicine, community health, care coordination preferred.

Expected hours for this role are 37-40 hours per week with few opportunities for overtime.

Typical work hours are flexible, but traditional office hours are preferred, 8 : 00 AM - 5 : 00 PM.
