Congreso De Latinos Uni is Hiring a Student Success Case Manager Near Philadelphia, PA

 

The Student Success Case Manager i.e Truancy Case Manageris responsible forcollaboratingwith school personnel and families to support students in improving attendance and developing positive patterns of school engagement, with the goal of avoiding or discharging the family from Truancy Regional Court and achieving academic success. Truancy Case Managers are paired with children who have three or more unexcused absences for up to 90 days of service. From within the child’s school, the Truancy Case Manager works closely with children and their families to assess and remove barriers to attendance, including providing education on attendance policies, connecting families to helpful resources and services, providing emergency supplies or incentives, and increasing the student’s motivation to attend school. For students with ten or more unexcused absences, the Truancy Case Managers helps the family navigate through judicial system including Regional Truancy Court, and rarely, Family Court appearances. This work involves conducting a thorough assessment of the strengths and needs of the families, developing a service plan, and implementing problem-solving interventions, including coordinating warm handoffs to community programs or services. Prevention Case Managers guide families through the process of enrolling in supportive programs using a strengths-based approach and respecting the family’s self-determination. This work also involves supporting families through advocacy and providing essential resources.

CASE MANAGEMENT

  • Provide school-based attendance support by removing barriers to attendance for students on caseload. 

  • For Tier 1: work with students who have six or more unexcused absences and have had a Student Attendance Improvement Conference (SAIC) for up to 90 days of service. Attendance Case Managers provide intervention to remove attendance barriers by engaging families, assessing needs, increasing confidence and motivation for success, provide education, resources, and connection to services. 

  • For Tier 2 or 3 services: conduct outreach to the child’s home and school within time frames dictated by funder. Engage children and their families, complete assessments and provide relevant education and follow-up support to childrenfamilies, and schoolsAttending all Truancy-related court hearings alongside family. Continue to provide monthly support to the child and family through home and school visits. Continue to support the family until discharged fromthe program successfully or transferred to more intensive services.

  • Safeguard the confidentiality of client families. 

  • Promptly report all conflicts among client families, or serious incidents involving clients and families within 24 hours of the incident. 

  • Provide services and support outside of working hours to accommodate the family’s needs when necessary. Be available on-call 24/7 for client emergencies (occasional). 

  • Regularly attend and actively participate in staff meetings, supervision, staff training sessions, case reviews, etc.

  • Perform other duties as assigned. 

COMPLIANCE & QUALITY ASSURANCE

  • Adhere to documentation deadlines and be able to express themselves clearly in writing. Case Managers are to submit written testimony in the form of a Court Report for all hearings. Case Notes are to be entered into digital database systems within 5 days of the contact event. All Case Managers will also maintain a neat and tidy paper file for each case. Ability to navigate the internet and digital data systems required.

  • Prepare for and attend supervisory support meetings including one-on-one supervision and team meetings. Staff will also attend training courses and meetings presented by the Department of Human Services, The School District of Philadelphia, Congreso, and other relevant partner agencies. This position will also require professionalism and the ability to positively engage a variety of stakeholders and build professional networks.

  • Keep quality case files that pass all internal and funder file audits. 

  • Complete assessments, home visits, and other required activities within the standard timeframe. 

  • Ensure that program quality assurance tools are used and updated according to deadlines. 

  • Perform related duties as assigned. 

Minimum Experience, Education, and Qualifications

Minimum Education: Bachelor’s degree in Social Work or related field required from an accredited college/university. Examples of related fields include social services, sociology, psychology, theology, education, criminal justice, human services, and public administration.

Minimum Experience:

  • 1-3 years’ related experience required. 

  • Knowledge of the community and resources throughout the city 

  • Computer proficiency in Microsoft Office Suite is required.

Certification/License: 

  • Mandated Reporter training certificate required and may be completed post-hire. 

  • Valid driver’s license or state identification required. 

  • Criminal and child abuse background clearances must be obtained before onboarding.
