JOB DESCRIPTION
Provide skilled PT services / intervention in order to
- Restore and / or maintain Residents / patient's function, alleviate pain, and prevent disabilities by planning and administering medically prescribed physical therapy.
- Provide quality care by assessing and interpreting evaluations and test results.
- Determine physical therapy treatment plans in consultation with physicians or by prescription.
- Helps patient accomplish treatment plan by administering a variety of therapeutic modalities and procedures, and using equipment as necessary.
- Evaluate, treat and record Residents / patient's progress, modifying treatment plans, and trying new treatments if required.
- Evaluates the use and fit of prosthetic and orthotic devices by observing, assisting, providing inputs, observing residents / patient's progress and recommending adjustments and modifications.
- Screen / Assess residents for potential benefit from skilled PT intervention.
- Documents resident / patient care services by charting patients and department records.
- Instructs Residents in proper in-home exercises and therapies.
- Educate residents / patients and their families about the recovery process and the challenges they will face.
- Supervise physical therapy Assistant and other Rehab personnel.
- Participate in Department and Interdisciplinary meeting.

License / Certification :
- NYS Physical Therapy License
- Basic Life support (BLS) Certification
- 0-3 years of experience, new graduates, welcome (will train)

Schedule :
- 8 hours shift
