POSITION SUMMARY The Clinical Supervisor is a licensed health care professional responsible for overseeing day to day patient care by assisting patients, caregivers/families, Interdisciplinary Group (IDG) members and health care providers involved in patient care. This is a supervisory role to ensure patient care is coordinated and managed efficiently and with the highest quality in accordance with state and federal guidelines. QUALIFICATIONS Current Registered Nurse (RN) license in the state of PennsylvaniaTwo (2) years hospice experience preferredPossesses strong assessment, problem solving and decision making skillsSolid computer skills, including Word, Excel and electronic medical record (EMR) programsExcellent organizational skills with attention to detailOutstanding communication skills (both written and verbal)Must be able to identify and anticipate patient and family needsMust possess the ability to coordinate with scheduling sufficient patient coverage on evenings, weekends and during vacations and holidays RESPONSIBILITIESTriages and provides assistance to patients, caregivers/families, Interdisciplinary Group (IDG) members and health care providers as it relates to patient problems, concerns, and/or questionsAssists IDG members with interventions and care as outlined in the individualized patient plan of careCollaborates with hospice medical directors and obtains appropriate physician orders and prescriptions as necessaryWorks closely with Intake and scheduling to assure timely patient care and admissionsAdheres to regulatory processes as it applies to patient/family care (Certificate of Illness (CTI), plans of care, IDG, order management, Additional Documentation Requests (ADR) and Department of Health (DOH) monitors)Coordinates IDG meetings; assists with patient schedules and required visits in order to remain compliant with DOH regulationsCompletes all computer and/or paper documentation requirements within the timeframes specifiedParticipates in required on-call responsibilitiesAttends and participates in staff meetings and other related activities when requiredDemonstrates an understanding of the QAPI process, hospice policies and procedures, and participates in projects when requestedServes as a guide and navigator to field clinicians in order to achieve productivity and customer-friendly careWorks efficiently with other team members to coordinate duties and assure coverage for all tasks including but not limited to: answering patient/family calls, completing new admission workflow, monitoring patient medication lists, reviewing the CTI/coding, reviewing recertification visits, contacting patients/families following triage calls, coordinating care plan meetings with team, assisting the RN Case Manager to initiate POD meetings, communicating with team, ordering/refilling prescriptions/medications, reviewing IDG notes for team meetings as well as checking for completion, working with facility staff for collaboration of hospice care, coordinating discharges and revocations for hospice care, providing guidance and assistance to coordinate supplies for patients and field clinicians
