The Clinical Management Assistant manages, monitors, and evaluates the care and service of the patient from admission to discharge. RESPONSIBLE TO Clinical ManagerExecutive Director QUALIFICATIONS Two (2) years of progressive experience in a home health care setting preferredInterpersonal, communication and organizational skillsProficient and knowledgeable of an electronic medical recordKnowledge base of agency policies, federal and state regulations and third-party payersReliable, independent means of transportation. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state RESPONSIBILITIES Reviews clinical documentation for compliance and completeness in a timely manner.Reviews OASIS for accuracy and completeness. Notifies the primary clinician of the need for recertification/discharge decision in a timely manner.Knowledge about payer processes and requirements to bridge the gap between provider and payer and to effectively meet the needs of both, ultimately for the patient’s benefit.Facilitates communication between patient/family, physician and all other healthcare providers.Assimilate data for physician practices and others as requested.Other duties as assigned DOCUMENTATION Exhibits a thorough knowledge of the agency’s electronic medical record documentation system. Able to demonstrate expertise in all aspects of required documentation completion. COMMUNICATION Ensures the accuracy of information being communicated with physicians, other health care providers, and staff.Interacts with staff, patients, families, physicians, other health disciplines, agencies, insurance companies, hospitals, labs, employers, government agencies and representatives as necessary.
