Description

Comunilife Inc. is looking to hire an experienced Social Worker to join its Bronx Congregate program, EL Rio II. The Social Worker will conduct psychiatric and other assessments, as well as develop and implement person-centered treatment plans, and supportive services. Services are delivered through evidence-based client treatment practicesThis position is eligible for Comunilife's generous benefits and paid time off package including medical, dental, vision, life insurance, retirement employer contributions after 2 years, tuition reimbursement and professional development opportunities.
Essential Duties:
  • Assess clients’ substance abuse, health, mental health, and other care needs; recommend internal and external referrals; 
  • Provide individual and group counseling;
  • Prepare psycho-socials;
  • Facilitate psycho-education groups in relapse prevention, health literacy, and navigating mental health and substance abuse systems, etc. for residents their family and/or support networks;
  • Provide Comprehensive Case Management services;
  • Assist clients in improving and maintaining self-care, interpersonal, and vocational skills 
  • Assist clients in achieving housing stability 
  • Manage client documentation, provide program narrative and/or statistical reports as needed;
  • Participate in ongoing QI and program evaluation activities

Qualifications


Education and Experience Requirement(s):
  • Master’s degree in Social Work (MSW) required; New York State Licensed Masters in Social Work preferred; 
  • Two (2) years’ experience with homeless populations; 
  • Two (2) years’ experience with persons with chronic health and/or mental health conditions and/or substance abuse disorders;
  • Bilingual/Spanish skills a plus
