Description

Comunilife is looking for experienced Case Managers to fill open positions for their El Rio Residence Program in the Bronx. The Case Managers main responsibility is to provide supportive services to assigned program participants in a housing setting. The type of support includes assisting with defining personal, vocational, and educational goals and using a person-centered / Harm Reduction model that supports individuals in their recovery, hopes, and opportunities. Essential Duties

Ensure that each client is receiving appropriate and effective case management services.

Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals.

Conduct face to face contacts (home visits/office visits) as specified in the service plan(s); with a minimum of one home visit and one contact per week.

Provide assistance in accessing primary medical care; assistance with adherence to medication regiments; teach activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training, and private sector placement services; prevention with positives; and other necessary services to clients to increase their capacity for independent living.

Conduct reassessments and review service plans as needed on a quarterly basis to ensure that services are appropriate for the clients current needs.

Ensures that each client has psychosocial.

Provide crisis intervention to clients and their families as needed.

Maintain ongoing awareness of clients medical conditions so that services are appropriate to changing needs.

Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services.

Provide counseling and orientation regarding tenants rights and responsibilities to maintain and retain housing.

Assist clients in moving from hospitals, nursing homes or other locations into their apartments, and provide transportation when necessary.

In collaboration with vocational counselors and entitlement specialists provide advocacy & referrals to educational, training and/or employment programs and/or job placement in the private sector.

Assist clients in locating alternative housing options, when a clients health so requires.

Provide and coordinate all supportive services as needed.

Collaborate with all providers within and outside of Comunilife involved with the client/family.

Maintain regular personal contact, developing a trusting working relationship with clients in order to monitor physical and emotional capacity for independent living.

Maintain and complete required documentation including all contacts made with the client and any contact on their behalf.

Participate in weekly supervision.

Submit a weekly schedule of field visits.

Participate in mandated training workshops, seminars, and case presentations as well as meetings with peers and supervisors.

Qualifications

Bachelors Degree required; with minimum two years field work experience and direct mental illness work experience. Individual, family and group counseling experience required, particularly with mentally ill populations. Preference given to persons with experience in working with homeless individuals in Congregate Programs, bi-lingual, bi-cultural in Spanish, a plus.
