NOW HIRING: Social Worker

Social Worker Responsibilities:

  • Provide psychosocial support to residents and their families, addressing emotional, social, and psychological needs.
  • Conduct assessments to identify residents' strengths, challenges, and goals for care.
  • Develop individualized care plans in collaboration with interdisciplinary team members, including nurses, therapists, and physicians.
  • Advocate for residents' rights and coordinate with community resources to ensure access to services and support.
  • Facilitate family meetings and support groups to address concerns, educate families, and promote effective communication.
  • Assist residents in navigating transitions, such as admission to the facility, discharge planning, and end-of-life care.
  • Document assessments, interventions, and progress notes in compliance with regulatory requirements.
  • Collaborate with facility staff to promote a positive and supportive environment for residents and their families.
  • Participate in interdisciplinary team meetings and case conferences to coordinate care and address complex issues.
  • Stay updated on best practices, ethical guidelines, and regulations relevant to social work in long-term care settings.

Social Worker Qualifications:

  • Bachelor's or Master's degree in Social Work or related field.
  • Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) in the state of New Jersey.
  • Previous experience in a social services leadership role in a long-term care or healthcare setting.
