Responsibilities:
- Oversee and coordinate all social services functions within the nursing home.
- Conduct psychosocial assessments and develop individualized care plans for residents.
- Provide counseling and support to residents and their families to address emotional, social, and financial concerns.
- Collaborate with interdisciplinary team members to ensure comprehensive and holistic care for residents.
- Ensure compliance with state and federal regulations related to social services in long-term care facilities.
Qualifications:

- Bachelor's or Master's degree in Social Work or related field.
- Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) in the state of New Jersey.
- Previous experience in a social services leadership role in a long-term care or healthcare setting.
