Compass Healthcare Consulting & Placement is conducting a search for a bilingual Assessment RN for a Managed Care Plan servicing New York. 8-12 visits per week, depending upon position. Will consider consider candidates with home care or UAS / MLTC experience. Bilingual Languages a Plus! Qualified candidates will be able to visit members in their homes. Available areas include: Manhattan, The Bronx, Queens, Nassau County, Rockland County, Orange County, Ulster County, Duchess County, Putnam County or Sullivan County. Qualified candidates will have prior UAS/MLTC RN experience or a minimum of 3 years of Home Health RN experience, reliable transportation, a NYS RN License, prior community health, managed care and/or UAS assessment experience.
Summary:
The Assessment RN will independently conduct functional, psychosocial and environmental safety assessments, determine eligibility of enrollees, and establish an individual plan of care that enables them to remain safely at home despite long-term care needs. Great listening and communication skills with a culturally diverse population that includes caregivers as well as other providers along with time management skills will be key to success.
Qualifications:
Current NYS RN license and registration
Associate's Degree
Minimum of one year experience in Community Health, Managed Care or related field; UAS Assessment or ReAssessment experience preferred
Working knowledge of Medicare, Medicaid and NYS DOH standards and regulations
Bilingual Spanish, Cantonese, Mandarin, or Russian a plus!

Salary range: $98,000.00-105,000.00
