• In collaboration with the Restoration Services Manager and Hospital-Based Interventionist, assist families in creating a hospital discharge plan for people impacted as well as conduct a safety assessment with the family
• Maintain consistent contact - including phone calls, texts, and in person meetings - with each family in accordance with their phase: 
• Phase One (72 hours): Conduct safety planning, hospital discharge plan in collaboration with family, Community Safety Team, and Hospital-Based Interventionist
• Phase Two (2-4 weeks): Daily communication, 2x per week in-person contact
• Phase Three (2-3 months): Every other day communication, 1x per week in-person contact
• Connect family members with available community-based well-being and mental health resources
• Connect families with available public aid including, but not limited to: Medicaid, SNAP, TANF, victim services, etc.
• Support vigils, funerals, and memorial services as needed in collaboration with the Restoration Services Manager
• Meet with the Care Team (Resource Navigator, Street Team Member) for meetings and team case review
• Attend regularly scheduled supervision meetings with the Restoration Services Coordinator to review workload, highlight successes and brainstorm solutions to any challenges that arise
• Attend all mandatory meetings, required training, and professional development opportunities
