Overview:
We are seeking a compassionate and dedicated Social Worker to join our team. As a Social Worker, you will play a vital role in providing support and assistance to older adults and their families. The Community Partnership on Aging is a unique collaboration between six cities (Highland Heights, Lyndhurst, Mayfield Heights, Mayfield Village, Richmond Heights, and South Euclid) that serves older residents in those cities and throughout Northeast Ohio. Community Partnership on Aging seeks a full-time social worker to perform duties including supportive services, information dissemination, referral to community resources and benefit counseling to senior residents of Partnership cities and their families.

Essential Functions:

· Conduct client-find/assessment interviews, developing an appropriate client care plan, if indicated and as appropriate.

· Ensure client care plans are reassessed at specified intervals, per program requirements, and as needed, for clients who are actively participating in agency services.

· Provide supportive services, outreach advocacy, information and referral, and other social services as appropriate. Make referrals to other agencies, as needed.

· Provide individual and family supportive counseling as indicated by care plan. Make referrals for therapeutic counseling to local providers as appropriate.

· Complete written case notes, accurately recording client contacts, and recording coordination with family and with other agencies on clients’ behalf.

· Maintain up to date knowledge of available services within each of the member cities.

· Participate in special programming and activities offered by CPA during off hours, with advance notice.

· Assist in field education for any student training at CPA.

· Attend continuing education workshops and maintain up to date license.

Cognitive/educational requirements

· LSW/LISW required

· Ability to solve problems independently and handle emergency situations

· Display sensitivity to aging population, experience a plus

· Demonstrate ability to establish and maintain positive relationships with clients, colleagues and community partners

· Ability to use agency software system in recording, maintaining and reporting data Proficient in the use of Microsoft Office 365 (Word, Excel, Outlook)

The Community Partnership on Aging requires applicants to complete a pre-employment screening process upon receipt of an offer of employment. CPA will cover the cost of a fingerprint background screening and drug screen after an employment offer is made. Any employment offer is contingent upon receipt of all satisfactory pre-employment screenings.

Pay: $22.00 - $24.00 per hour
