Community Healthcare System is hiring a LPN for the Emergency Department

Under the direction of the Registered Nurse, the LPN is responsible for implementing, directing, communicating and documenting the nursing process and provides a safe environment for patients, visitors and associates. The nurse practices in accordance with current established standards of care, the Nurse Practice Act, the Community Healthcare System policies and procedures, and supports the mission and philosophy of CHS by providing the highest quality care in the most cost efficient manner, provides for the well-being of the community, services the needs of all people and demonstrates the values: dignity, compassionate care, community, quality and stewardship.

 LPN Roles and Responsibilities:

• Practices under direction of RN while following all policies/procedures/standards related to care by the Licensed Practical Nurse (LPN) of Community Hospital and Indiana Nurse Practice Act.
• Assesses patients per patient acuity, needs and unit Scope of Care
• Independently identifies nursing diagnoses and develops individualized plan of care based on assessment findings.
• Communicates assessment findings and pertinent patient information to physician.
• Demonstrates effective knowledge of electronic medical record. Documents effectively to reflect accurate account of care given.
• Coordinates, educates and evaluates plan of care, directed at patient progression toward discharge.
• Demonstrates clinical competence and seeks resources as needed.
• Demonstrates safe medication administration practices.

Required Skills & Qualifications:

• Graduate of an accredited school of practical nursing.
• Current professional Licensure by the State of Indiana Health Professionals Bureau.
• Successful completion of The Community Healthcare System’s Personnel and Nursing Orientation Programs.

• Obtains and maintains current certification as required in specialty areas.
• Obtains and maintains current CPR certification
