Job Summary:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will be responsible for coordinating and overseeing the care of patients, ensuring they receive the appropriate services and resources needed for their well-being.

Responsibilities:
- Manage a caseload of patients, providing comprehensive case management services
- Collaborate with healthcare providers, social workers, and other professionals to develop and implement care plans
- Conduct assessments to identify patient needs and develop individualized care plans
- Monitor patient progress and adjust care plans as needed
- Provide counseling and support to patients and their families
- Supervise and coordinate services provided by other healthcare professionals
- Maintain accurate and up-to-date patient records

Qualifications:
- Bachelor's or Master's degree in Social Work, Psychology, Counseling, or related field
- Previous experience in Behavioral Health, Managed Care, Addiction Counseling, or related field preferred
- Knowledge of medical terminology and patient care practices
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Experience in Hospice Care, Acute Care, or similar settings is a plus

Pay: $40,000.00 - $47,000.00 per year
