Community Care is hiring a Case Manager/Care Manager to join our Family Care Program Team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission driven organization. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. It is based in Racine County, with telework currently being the norm. Case Manager/Care Manager Job Responsibilities: Manages an assigned caseload of program members in collaboration with an RN Care Manager Completes comprehensive member assessments, member centered care plans, & functional screens Provides ongoing care coordination and quality assurance monitoring of services Maintains required documentation in adherence to established standards. Case Manager/Care Manager Job Requirements: Education: Bachelor’s Degree in a Human Services Area required. Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity required. Certification: Eligibility for long-term care functional screener certification is required Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage. Care Manager Job Schedule: Monday to Friday Generally during business hours of operation (8AM-4:30PM) Community Care is a nonprofit organization with over 40 years of experience helping adults with long-term care needs live as independently as possible. Our Medicare/Medicaid long-term care programs serve the needs of older adults and adults with disabilities. We coordinate and deliver a full range of supportive services that help them live safely, confidently and with dignity.
