Essential Duties and Responsibilities:

Outreach, Intake and Case Management:

· Provides direct emergency and supportive services to clients in a variety of settings

· At intake, interviews clients to determine eligibility and to assess the needs of the client and their family

· Develops case plans with clients and assists clients in developing goals and case plans.

· Facilitates client access to other programs within Community Action Partnership of Natrona County and to other community resources and agencies

· Monitors case plan, including comprehensive tracking of client services and outcomes

Networking

· Represents the agency and its clientele to the community, public and private agencies by serving on boards or committees, attending community events and maintaining working relationships with a broad range of agencies and organizations.

Administrative

· Maintains records and files and submits reports as required

· Maintains comprehensive client files

· Maintains client documentation by completing forms necessary for clients to maintain compliance status, compiling progress reports and entering information into a database

· Utilizes intuition and experience to compliment data. Demonstrates effective/technical expertise in area of responsibility. Works with established systems and processes. Effective and flexible in implementing new systems and procedures. Keeps abreast of changes and requirements.

· Duties as otherwise assigned

Supervisory Responsibilities:

This position normally does not require supervision of other employees.

Qualifications Requirements:

To perform this job successfully, an individual must be able to perform each essential duty satisfactory. The requirements listed below are representative of the knowledge, skills and ability required.

  • Considerable knowledge of the economic, social and cultural needs of the clients served at CAPNC
  • Considerable knowledge of community resources
  • Ability to assess social and economic problems and assists clients in developing long-term solutions
  • Ability to understand all relevant federal and state laws, regulations and operating guidelines
  • Ability to accurately maintain required records
  • Ability to organize and prioritize work
  • Interpersonal skills necessary to work well with individuals and families
  • Skill in deescalating situations with angry, upset or hostile clients
  • Proficient working knowledge of Microsoft Office programs including Word and Excel

Education and Experience:

Work Record:

Employee will have a demonstrated work record showing good attendance, punctuality, dependability and the ability to work well with supervisors, co-workers and program participants.

Language Skills:

Employee has strong verbal and written communication skills to convey information and ideas, and influences through effective persuasion. Employee will have an ability to read and interpret documents such as safety rules and operating/organizational policies and procedures and an ability to write routine reports and correspondence.

Mathematical Skills:

Employee will have an ability to calculate figures and amounts such as sliding fees, proportions and percentages and apply basic concepts of algebra. Mathematical accuracy and attention to data are essential skills for this position.

Reasoning Ability:

Employee researches and utilizes resources to solve problems and develops and prioritizes alternative solutions. Employee will Exhibit sound and accurate judgment and an ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Employee can interpret a variety of instructions furnished in written, oral, diagram or schedule form.

Team Work:

Employee will have an ability to work and interact effectively and positively with other staff members to build and enhance teamwork in the overall CAPNC Organization. This position requires individual flexibility, initiative and the ability to handle several complex tasks at one time.

Sensitivity to the Needs of Groups with Special Needs:

Employee will have an ability to understand and respond appropriately, effectively and sensitively to individuals with special needs that are served by Community Action Partnership of Natrona County. Special need groups include those defined by race, ethnicity, language, age, sex, etc.

Physical Demands:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to drive, stand, sit, handle papers, type and operate computer equipment, reach with hands and arms, talk, see and hear.

  • A person in this position must be able to hear, see and speak English clearly.
  • Must be able to travel by auto and air for various meetings. A valid Drivers License is required.
  • Out of office travel may be necessary, including overnight travel
  • Minimal lifting (files, office supplies, etc to a maximum of 25 pounds.)

Work Environment:

Work is performed in a typical business office environment and periodically at locations outside of the office. Attends seminars, conferences and pursues similar educational opportunities for professional development. Some trips by automobiles to other sites in Wyoming and by airplane to other sites outside of Wyoming may be required.

Employee at Will:

This position is an Employee at Will position. Employment may be terminated at any time, with or without cause, unless specifically modified by a written contract of employment or statutory provision.
