ESSENTIAL DUTIES AND RESPONSIBILITIES

(Duties assigned to this job include, but are not limited to, the following examples. Any one position may not perform all listed duties or may perform other duties as assigned.)

  • Assist with the implementation of collective the reentry /stabilization initiatives, and other cross-community collaborative programs, addressing the needs of individuals experiencing chronic homelessness, formerly incarcerated, behavioral health conditions (including but not limited to substance abuse), or impacts resulting in high utilization of emergency services without remedy
  • Ability to assess short-term and long-term needs and engage in community, multi disciplinary program to address case staffing, planning and deployment of plans for individual that are case managed, with fidelity.
  • Advocates for the client to facilitate positive outcomes.
  • Act as point of contact for legal system and social service agency referrals as related to client needs.
  • Gather and enter statistical data for multiple programs, prepare reports on outcomes, both qualitative and quantitative; must be timely, complete across all file/data sets required
  • Provides reliable and consistent presence at community outreach events that connect and promote active communication between target population served and community organizations, vendors, landlords, local task force, and other relevant agencies; establishes "warm handoff' of individuals being served and creates environment of team support across provider network
  • Assist clients in completing the necessary documentation to access additional community resources and benefit linkages to meet basic human needs
  • Assist clients in acquiring ongoing income sources such as Social Security, disability, employment and other financial resources; including SOAR

certification support.

  • Promotes the optimal allocation of monetary and non-monetary resources through effective, appropriate and efficient application.
  • Effectively communicate with supervisor on significant developments and progress of assigned cases; receive coaching to deploy intensive case management to clients which includes accompanying clients to various appointments, arranging transportation for clients and conducting home visits.
  • Work cooperatively with the detention agencies ensuring all policies and procedures are adhered to, working in secure facilities with inmates providing intakes, briefings, case management and other services.
  • Implements housing initiative guidelines with internal case management staff, community housing collaboratives and ensures participants adhere to program guidelines, when placed.
  • Identify resources and provide referrals as necessary for stabilization, relevant to case plan.

Other Responsibilities:

  • Performs additional duties as assigned.
  • Follow and maintain knowledge of federal HIPAA guidelines.
  • Maintain professional, accurate, case files
  • Attend assigned meetings, trainings, and conferences.
  • Work with other staff members as directed by supervisor to sustain office functions and continuity.

MINIMUM QUALIFICATIONS AND EXPERIENCE

Considerable knowledge of trauma informed case management practices, including principles of harm reduction, re-entry, chronic homelessness, substance use/abuse and poverty; reasonable knowledge of resources relating to resource coordination for target population; reasonable knowledge of community networking; understanding of barriers related to poverty, highly desirable to for individuals who demonstrate knowledge equivalent to at one year of intensive case coordination for vulnerable persons.

: Writing clear and concise correspondence, records, reports and statistical data; promoting public relations and partnerships. Must be able to set clear boundaries and ensure that program compliance best practices are observed without exception. Highly desirable are skills in trauma informed care, adverse child experiences (ACE's) impacts on adults and outcome based planning.

Communicate effectively in both oral and written form; lead, support, motivate and train volunteers; develop training materials; relate to individuals and respond to their needs; make decisions independently in accordance with established policies and procedures, and use initiative and judgment in completing tasks and responsibilities; work within team concept; utilize problem identification and resolution techniques; work independently and manage time efficiently; remain calm and use good judgment during confrontational or high pressure situations; courteously meet and deal effectively with other employees, volunteers, program sites, outside agencies, committees, and the public.

Job Type: Full-time

Pay: $21.00 - $25.00 per hour

Expected hours: 40 per week
