Job Responsibilities of an LVN:
- Effective Communication: Proficient in communication with physicians, patients, and families, capable of relaying sensitive information effectively.
- Clinical Documentation: Assessing and evaluating patient status, accurately documenting clinical notes via electronic charting, ensuring comprehensiveness and accuracy.
- Collaborative Care: Implementing current nursing practices based on patient assessments and approved care plans initiated by Registered Nurses, ensuring continuity and quality of care.
- Patient Education: Assessing and providing patient and family/caregiver education pertinent to diagnoses and care plans, empowering them with relevant information for better understanding and management.
- Treatment Evaluation: Assessing patients' response to treatments and medications, ensuring appropriateness and effectiveness, and promptly reporting any concerns or changes.
- Task Delegation and Supervision: Capable of delegating tasks effectively, overseeing their completion to standard, and providing necessary guidance or correction as needed.
- Timely Documentation: Ensuring timely submission of all documentation, maintaining accuracy and completeness, facilitating effective communication among healthcare team members.
CREDENTIALS/REQUIREMENTS:
Current California License as a LVN with 1 year of Hospice experience required.
Professional license in California and clinical school diploma
Current CA drivers license
Valid social security card or passport
Professional liability insurance if possible
CPR
Current physical/TB results (within recent 6 months) and flu vaccine record
Auto insurance
