Colorado Health Network is Hiring a Care and Housing Case Manager Near Grand Junction, CO

Position: Care and Housing Case Manager
Classification:Non-Exempt
Reports to:Client Services Manager
Location:Grand Junction, CO (occasional statewide travel will be required)
Starting Salary:$41,925(3 years of experience or less)
$43,939 (3 years of experience)

COVID-19 Information: It is an essential function and business necessity of the job that all employees are vaccinated and that all new hires will be required to be vaccinated within four weeks of hire. Position Summary:
This role oversees two distinct responsibilities. As a Medical Case Manager, the position provides tailored case management and advocacy to approximately 30 clients living with HIV/AIDS in western Colorado. Additionally, it manages housing assistance for approximately 15-20 eligible households through the Tenant-Based Rental Assistance (TBRA) and Shallow Rent Program (SRP).

Essential Job Functions:

Job Functions:

  • Provide case management for a caseload of clients, assessing physical and psychosocial needs through personal visits, phone calls, and written correspondence.

  • Conduct comprehensive intake and needs assessments, addressing healthcare access, medication adherence, income stability, mental health, substance abuse, nutrition, and HIV prevention.

  • Develop Individual Wellness Plans with clients to address identified needs, ensuring timely updates according to program policies.

  • Document all client interactions accurately and timely, collecting eligibility documentation as required.

  • Assess client acuity monthly and manage case inactivation based on service usage and program policies.

  • Identify and refer clients to internal and external resources as needed, coordinating with various departments and community providers.

  • Coordinate all occupancy aspects of multiple scattered-site, subsidized housing programs, including Tenant-Based Rental Assistance (TBRA) and Shallow Rent Program (SRP).

  • Schedule and perform Housing Quality Standards inspections annually, following up with tenants and landlords to remedy deficiencies.

  • Serve as liaison and technical support for other Colorado Health Network locations participating in housing programs, including vacation coverage for counterparts in different physical locations of the state.

  • Coordinate database updates and distribute checks for the TBRA program.

  • Develop and maintain resources for staff and clients in affordable housing.

  • Collaborate with case management to ensure housing accessibility and develop long-term housing plans for program participants.

  • Track and submit appropriate documentation, billing, and reports to the Housing Programs Administrator and grantors.

  • Develop and maintain ongoing working relationships with local landlords, housing authorities, and affordable housing providers.

  • Participate in team meetings, case conferences, staff meetings, and community functions representing the organization.

  • Assist with agency program audits and serve as a representative of the regional office at HIV medical clinics and community functions.

Required Qualifications: Valid driver’s license and vehicle with current auto insurance required. Some travel throughout the Western Colorado region required. Must be able to work in a diverse setting with diverse populations, including sexual orientation. Background in programs related to affordable housing, sexual health, risk reduction, financial assistance, and other resource programs, especially as related to serving People Living with HIV, required.

Preferred Qualifications:
Bilingual (English/Spanish) highly desired. 2 years case management experience and training or 3 years Promotora experience preferred. Training in cultural competency/sensitivity is a plus. Experience working with federally funded programs desired.

Education:
Bachelor’s degree in Human Services, Social Work, Psychology, or related field preferred. Candidates with lived experience can substitute educational experience.

Software Utilized:
Experience working with basic office and database computer programs, including familiarity with Microsoft Outlook, Word, Excel, Access, SharePoint and the internet.

Knowledge, Skill and Ability:

  • Incredible time management skills.

  • Strong communication, client relations, and managerial abilities.

  • Preferred knowledge of HIV and related issues.

  • Familiarity with the Harm Reduction Model and Motivational Interviewing is desirable.

  • Cultural competency and sensitivity are essential.

  • Demonstrated flexibility, creativity, collaboration, and a positive workplace demeanor.

  • Understanding of financial management and budgeting principles.

  • Exceptional organizational skills with a keen eye for detail.

  • Personal and professional accountability, self-motivation, and initiative are required.

  • Commitment to quality improvement, professional integrity, research ethics, and cultural competency.

  • Ability to work accurately and efficiently while demonstrating excellent organizational skills.

  • Effective collaboration within small and large teams, both locally and regionally.

  • Proficiency in task management, teamwork, time management, and communication with colleagues, supervisors, clients, and external consultants.

Material and Equipment Directly Used:
Works with standard office equipment including computer workstation, printers, copiers, scanners and fax machine. Works with word processing software, spreadsheet applications, and presentation software. Communicates with clients and vendors using the telephone, e-mail, the Internet, and fax.

Physical Demands and Work Conditions:
Works in an office environment. Requires clear speaking ability and the ability to use a telephone for extended periods. Requires ability to view a computer screen (near acuity). Requires fingering agility for keyboarding and using a telephone and other office equipment.
