Community Hospital is Hiring a Primary Care RN Case Manager, Full-Time Near Grand Junction, CO

RESPONSIBILITIES:

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Maintain and improve the case management system using ongoing interaction with patients, physicians, and other health providers to meet designated clinical, operational, and financial outcomes for aggregate patient populations.
  • Demonstrate a solid understanding of all regulations and standards associated with Case Management that CWHS is held to.
  • Assist physicians, patients, families, staff, and payers in providing care for patient populations that is appropriate, effective, and cost efficient.
  • Attend and actively participate in all departmental and interdepartmental meetings related to Case Management/Social Services/Discharge Planning functions.
  • Assist in performing Medicare Annual Wellness visits.

EDUCATION and/or EXPERIENCE:

Bachelor’s degree in nursing (preferred) with at least three (3) years of clinical experience and at least two (2) years of case management experience; certification in case management (preferred)

OR

equivalent combination of education and experience

Current unrestricted Registered Nurse license in the State of Colorado (required)

Willing to obtain Case management certification within 3 years.

This position's hiring range is: $38.00 - 45.60 per hour, depending on education and experience.
