Collaborative Solutions for Communities is Hiring a ReEntry Program Manager Near Washington, DC

This position description incorporates the core responsibilities of the job. It is recognized that other related duties not specifically mentioned might also be performed and that not all responsibilities may be carried out depending on operational needs. Additionally, this position has flexible hours and a non-traditional work schedule.
This program addresses on an individual basis, the developmental, relational, and maternal needs of young mothers who are housed by the D.C. Department of Corrections (DOC) and are seeking to reconnect and be reunified with their children. The program facilitates a smooth transition from incarceration back into society, back into the lives of their family, and back into their roles as mother and caregiver. 40 young mothers who are incarcerated at Department of Correction's facilities in DC will be served. Second Chance, a partner on the project, will assist in identifying participants for the program and address the growing need for supportive counseling for incarcerated women at the Correctional Treatment Facility. The intervention with the children is intended to break the cycle of incarceration and antisocial behavior in children as they develop.
PRINCIPLE ACCOUNTABILITIES:
Clinical Services- Conduct on-going assessment of the family's strengths and progress towards independence or permanency goals. Effectively plans and implements interventions in the progress of case planning. Ensure that licensed supervision is provided for all clinical services. Make appropriate referrals to alcohol and substance abuse treatment, mental health services, and supports clients to access public benefit programs and based on assessment need and will of the client to engage in services.
Collaboration - Maintain relationships with other community agencies, educational institutions, businesses, and service providers to advocate for clients. Participate in internal and external meetings in spirit of mutual support of homeless individuals advocate for needed resources and services and to ensure clients are provided with all possible resources available.
Supervision - Ensure that all progress notes of engagement activities, client contacts, and clinical notation are recorded in the client electronic record within 48 hours of service delivery in either QuickBase or HMIS, depending on the nature of the information. Review files for each participant ensuring the requirements documents have been completed and documented. Review current skills of Case Managers, provide appropriate work assignments and case supervision. Identify strengths and deficiencies and develop a professional development plans with assigned staff.
Program - Adhere to all program requirements outlined in the grant. Submit a case management monthly report to the District by the 10th day of each month (reflecting activities for the previous month). The report should include all information outlined in the grant. Identify gaps in services to the participants and suggest methods or plans to address gaps in services. Ensure the appropriate annual trainings are completed and documented for program audit.
New Staff - Provide leadership, and actively engage new staff in developing knowledge of CSC, the strengths-based approach, and the framework in which community based social work practices are implemented. Acts as a resource for new staff by providing guidance and support when necessary.
Education
A master's degree in Human Services or Social Work, or bachelor's in Social Work or Psychologywith field experience.
Experience
3 years professional experience with re-entry programs or provisions of social services. Knowledge of community resources. Excellent oral, written and interpersonal skills.
Excellent problem solving, analytical, organizational, and oral and written communication skills. Ability to prioritize work.
