The Healthy Families Program is an evidence-based, voluntary family support program designed to promote positive parent-child relationships and healthy attachment between pregnant/postpartum caregivers and their families living within 50 miles of Flagstaff. The Healthy Families Program team consists of a Program Manager, Program Supervisor, Family Support Specialists and a part-time Family Resource Specialist. Family Support Specialists conduct weekly, biweekly, and monthly in-person home visits with a family-centered, relationship-based, and culturally-sensitive approach.
Under general supervision performs work of moderate difficulty in social services case management for at-risk clients; performs related work as assigned.

Typical Duties

(Illustrative Only)

  • Interviews and elicits pertinent information from persons applying for assistance
  • Conducts comprehensive assessments with individuals and families to identify strengths and areas of need
  • Interprets rules and regulations regarding eligibility for a range of programs
  • Works with clients to develop individualized case plans, including specific goals and success strategies
  • Provides referrals and information about other service providers, alternative assistance available, client responsibility and appeal rights
  • Supports clients in meeting short and long-term goals throughout the term of the case plan
  • Maintains and works with funding allocations
  • Provides follow-up and case plan review and revision as needed
  • Coordinates services with other agencies and makes presentations
  • May conduct home visits
  • Compiles data to prepare records and reports
  • Attends required meetings and trainings

Essential functions of this position include driving; working with and around other staff and clients; vision acuity (near) for reading computer work; hearing and speech for ordinary conversation and for communicating with staff and clients; finger dexterity / touch for typing filing etc.

Minimum Qualifications

Four years of experience of progressively responsible experience in social services work OR

Associate degree in a related field of working with children and families and three years of experience of progressively responsible experience in social services work OR

Bachelor's degree in a related field of working with children and families and two years of experience of progressively responsible experience in social services work OR

Any equivalent combination of education, training and experience which demonstrates the ability to perform the duties of the position.

The ideal candidate will have a bachelor's degree in a field related to working with children and families, a minimum of one year of experience providing services to children and families. The ideal candidate also has experience establishing trusting relationships and working with diverse populations.
Preferred Qualifications: Bachelor's degree, experience in providing services to children and families, bilingual in Spanish or Navajo


A
DDITIONAL REQUIREMENTS: Fingerprint clearance card, pre-employment drug screen and random drug screens are required. Additionally, upon hire candidates must show proof of current and valid motor vehicle insurance and it shall by updated after the expiration of the current policy.

This position is non-exempt from overtime.

  • Work well with people of diverse social, cultural, and economic backgrounds
  • Maintain strict confidentiality
  • Prepare and maintain accurate documentation and records
  • Work safely and support the culture of workplace safety
  • Establish and maintain effective working relationships with employees, other agencies, service providers, and the public
  • Follow written and verbal instructions
  • Communicate effectively verbally and in writing
