Position Description

Overview

The Nurse Practitioner reports to the Medical Director, Care Medical Practice (the Company), with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet the Company’s requirements. The Nurse Practitioner should embody the Company’s core values, including being: Trusted, Empathetic, Committed, Creative, Humble and Community-Minded.

Primary Responsibilities

The Nurse Practitioner will have the following responsibilities:

• In partnership with the clinical team of Care Medical Practice, PLLC, provide medical highest quality medical care for the practice’s patients.
• In partnership with the Medical Director and Management, assist and support clinical personnel (e.g., clinical nurses, allied health professionals, EMR, telemedicine operations).
• Provide consultations and perform diagnostic, therapeutic procedures in your clinical discipline within your practice scope, consulting when appropriate the supervising physicians for assistance with both diagnosis and treatment.
• Work collaboratively with a multi-disciplinary team to create individualized treatment plans for patients; embrace and promote a patient-centric model of care.
• Commit to the practice of value-based care which emphasizes care in the appropriate setting to improve and lower costs, address and close gaps in care and drive high patient satisfaction.
• Demonstrate proficiency and support use of the practice management system and electronic medical record (EMR).

General Duties

The Nurse Practitioner shall have following duties:

• Leadership: The Nurse Practitioner will lead in defining and executing strategies and solutions to create business value in our clinical practice, including building a team to design, develop, and execute those strategies and solutions to deliver desired outcomes.
• Strategy: The Nurse Practitioner will establish the business strategy and roadmap: (1) improve outcomes for the Company’s members; (2) enhance the efficacy of the practice; and (3) develop and deliver external market opportunities for products and services. In establishing the business strategy, the Nurse Practitioner will define and innovate sustainable revenue models to drive profitability of the Company.
• Collaboration: The Nurse Practitioner will ensure that our clinical capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of the practice.
• Knowledge: The Nurse Practitioner will provide subject matter expertise in the clinical solutions, including determining and recommended approaches for highest quality medical care, solution deployment and performance evaluation.
• Culture: The Nurse Practitioner is accountable for creating a productive, collaborative, safe and inclusive work environment for the clinical team and as part of the practice.

Qualifications

The Nurse Practitioner should have the following qualification:

• Education/Experience. A minimum of Nursing Degree or Nurse Practitioner Degree from an accredited U.S. school is required. Nurse practitioners must have completed accredited education and training for nursing and nurse practitioners. An active unrestricted medical license in the State of New York is required. At least 3 years of active practice required. DEA Certification required. 
• Entrepreneurial. Care Medical Practice seeks to fix gaps that have persisted for generations in the delivery of care to Black and Brown populations. The Nurse Practitioner will possess the ability to perform certain in-office procedures safely in an outpatient setting.
• Communication. Excellent verbal, written communication, and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to all stakeholders. Proficiency in all Microsoft Office applications.
• Relationships. Ability to build and effectively manage relationships with patients, business leaders and external constituents; and,
• Culture. Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial practice.
