Admission process, including transporting:

• Collects selected data as delegated by the RN, applies necessary equipment, labels patient belongings and checks Patient Belongings Form, informs patient/family of unit routines, etc.
• Obtains vital signs, blood glucose, identify belongings; ensure patient is wearing correct lD and allergy band.
• Set up rooms for admissions appropriate to the assigned unit. (i.e. Weight bed zeroed , suction set up with canister & tubing, IV pole and pumps, pulse ox, suction)
• Applies supportive equipment (TED hose, binders).
• Assists in the transportation of patients, including discharge. At discharge, checks belongings bag and Patient Belongings Form with patient
• Obtains and transports specimens to the lab and obtains blood and blood products from Blood Bank.

Direct Patient Care:

• Basic respiratory care, (i.e., incentive spirometry, pulse oximetry, O2 setup, coughing and deep breathing).
• Performs activities of daily living (ADL) appropriate with patient's condition, including hygienic care, patient repositioning, basic range of motion and ambulation.
• Provides nutritional support, such as the passing and collection of food and drink, patient preparation, feeding assistance, and documentation of intake as appropriate.
• Blood glucose checks as ordered by physician or nurse & document.
• Change wound dressings if not sterile as directed by nurse.
• Take vital signs, obtain 12-Lead EKG, apply cold/heat packs, I & 0 measurement, stage I decubitus ulcer care, DC heplock, collection of urine and stools, unmedicated enemas, oral suctioning, surgical preps and post-mortem care.
• Performs and documents daily weights as ordered.

Documentation and Reporting:

• Communicate with RN throughout day to accomplish any additional orders & tasks written during your shift.
• Communicates to the RN all pertinent observations concerning the patient's physical and emotional status.
• Intake and output throughout shift. (i.e. urine drainage device, colostomy, ileostomy, ensure hat/urinal is available for those who void, document incontinence, meal intake and oral intake.) Report any abnormal findings to nurse i.e. emesis, low urine output, no bowel movement.
• Obtains and documents vital signs.
• Documents nursing care given properly to patients.
• Reports equipment malfunctions to the Clinical Engineering department or maintenance and documents the report of the equipment malfunction.
• Maintains equipment and supplies in area of responsibility.
• Complete electronic work list each shift.
• Supports quality improvement activities on the unit through participation and knowledge on the assigned unit.

Ordering, Supplies, Maintenance:

• Maintains good housekeeping standards within assigned areas, i.e., patient rooms, nursing station, utility rooms, and where needed.
• Refrigerators checked daily and documented on log sheet.
• Bed making - occupied and unoccupied.
• Maintain clean and soiled utility rooms. (i.e. empty laundry bag as needed.)
• Monitor and maintain predetermined levels of supplies and equipment.
• Stock unit/rooms (i.e. closets, gloves, change needle boxes when full, blood glucose supplies, lab tray in clean utility, stock medication carts with syringes, medication cups, and etc.)

Qualifications:

• High school diploma or equivalent.
• Six months experience with direct patient care in an acute care or skilled nursing facility OR completion of an accredited certified nursing assistant program OR completion of one semester of clinical at an accredited associate or bachelor degree nursing program. 
• All applicants must successfully complete the PCA Orientation Program and PCA Evaluation Test.
• Required to maintain current BCLS certification. 
• Vaccination prior to starting.
