Purpose of this Position:

Children’s Service Center is currently looking for an energetic and dependable person who works well independently and collectively as a treatment team member. If you are ambitious and confident in your ability to learn, the Blended Case Manager (BCM) (similar to mental health worker, social services worker, targeted, Intensive case management, resource coordinator) position is right for you. You will be fully trained to work in a variety settings with a flexible schedule to closely monitor and advocate for services as needed by children/adolescents. You will report to the Blended Case Manager Supervisor. 

Responsibilities include, but are not limited to:

• Ability to use good judgement skills in assessing consumer needs
• Become familiar with available community resources (be up to date with their locations, eligibility requirements and program changes)

Qualifications:

Bachelor's degree in Social Work, Sociology, Psychology or related Social Science field

Two years experience or equivalent combinations of the experience and training in a public or private human services organization preferred, but not required
 
Experience in working with children/adolescents and their families is preferred, but not required 
 
Knowledge of the mental health and social service field preferred, but not required

Must pass Safe Crisis Management Training (physical and theory), upon hire and then annually as a condition of employment. Other requirements include:

• Criminal history
• Act 33 clearances
