What is the ACT Program?

Assertive Community Treatment (ACT) was created in the 1970s and is one of the oldest and most widely researched evidence-based practices working with severe mental illness adult clients. By providing person centered assistance including housing, recovery, employment support, legal support, D&A support, peer support, nurse/psychiatrist and therapist services, we enable clients to eventually transition to lower levels of care programs or move toward independence, increasing their quality of life. Clients may include adults with a history of trauma, homelessness, frequent hospital stays, and clients who have not responded well to traditional outpatient care.

ESSENTIAL FUNCTIONS:

  • Manages day-to-day clinical operations and required productivity standards.
  • Directs and coordinates consumer admission process, treatment and rehabilitation services, support services, and discharge process.
  • Facilitates team and treatment planning meetings.
  • Establishes and implements procedures for monitoring of workflow and promoting enhanced staff performance.
  • Create a strong sense of team accountability in order to achieve strong clinical and billing outcomes.
  • Maintain and assist in the development of procedures for provision of services within the ACT program.
  • Provides direct supervision of all team members.
  • Monitor service delivery and impact of service effectiveness as well as consumer satisfaction. 
  • Schedules staff work hours to ensure appropriate coverage, monitors and approves timesheets.
  • Assists with scheduling 24-hour crisis intervention on-call services on rotating basis.
  • Participates in staff recruitment, interviewing, hiring, orientation and performance plans.
  • Ensures the use of evidence-based treatments in the areas of mental health and substance abuse to address client needs (e.g., cognitive behavioral therapy, harm reduction, motivational interviewing, etc.)
  • Serve as practicing clinician to a small and assigned caseload of consumers. Serve as a practicing clinician to consumers on an as needed basis which includes providing crisis intervention, intake and needs assessments, etc.
  • Assist in the development of, monitoring, and maintaining the ACT Program annual fiscal budget.
  • Sets standards of performance by overseeing all documentation to ensure quality, timeliness and accuracy. 
  • Compliance with all PA DHS regulations, TMACT, Compliance and agency policy requirements.

Qualifications


The ideal candidate will possess the following qualifications:

  • Masters' Degree in related human services field of Social Work, Psychiatric Rehabilitation, Counseling, Psychology. etc.
  • Licensed Preferred but not a requirement.
    • Valid clinical license in a human service-related field such as an LSW, LCSW license in PA or transferable to PA. 
  • Skill and experience in group, individual and/or family counseling.
  • At least 3 years skill and experience in staff management, leadership, clinical supervision and program management experience is required.
  • Has a Peer Specialists supervisors' certification or will obtain this certification within 6 months of hire. 
  • This candidate also must have strong clinical skills and at least 3 or more years of post-master's clinical experience providing treatment to persons with severe and persistent mental illnesses.
  • This candidate must have a clear understanding of the characteristics and problems of adults with severe and persistent mental illnesses and is preferred to have knowledge about the ACT model and TMACT fidelity standards.
