Chestnut Health Systems is actively seeking a dynamic, full-time Community Health Specialist to spearhead and coordinate impactful projects supporting Prevention and Intervention plans across Logan County, Mason County, and McLean County. As a Community Health Specialist at Chestnut Health Systems, you will play a pivotal role in coordinating, implementing, and evaluating multiple projects across Logan County, Mason County, and McLean County. Your primary focus will be supporting Prevention and Intervention plans that includes travel within the service area (Lincoln, Havana, Bloomington, Normal), with potential for a hybrid work arrangement. Your responsibilities will encompass developing, executing, and assessing various programs, including Youth Prevention Education curriculum and evidence-based initiatives for both youth and adults. You will lead schools and community groups in developing evidence-based programs and practices by facilitating organizational activities, meetings, and member recruitment. Other key duties include managing program budgets, conducting outreach with community stakeholders, conducting needs assessments, crafting media campaigns, identifying funding opportunities, mentoring staff and interns, and actively participating in advocacy groups. As a crucial part of the team, you'll contribute to fulfilling agency and funder responsibilities while upholding Chestnut's high standards for customer service and safeguarding organizational information.
