Job Summary for Social Worker:

Identify the medically related social service needs of residents and ensure needs are met. This includes maintaining or improving their ability to manage their everyday physical, mental and psychosocial needs with the ability to relate appropriately to residents, families, staff, and general public. 

Qualifications:

• Bachelor’s degree in Social Work or in a human services field, including but not limited to sociology, rehabilitation, counseling, and psychology required
• Master’s degree in Social Work preferred
• Minimum 2 years of social work experience in a long-term care setting working directly with individuals
• Ability to relate appropriately to residents, families, staff and general public
• Genuine interest in the geriatric population 

Responsibilities: 

• Make arrangements for needed adaptive equipment, clothing, and personal items
• Assist residents and/or patients in making health care choices 
• Making and assisting with referrals from outside entities i.e. absentee ballots
• Discharge planning
• Assist with financial and legal matters i.e. referrals to lawyers, funeral home matters
• Promote actions by interdisciplinary staff that maintain or enhance each resident’s dignity
• Assist residents to determine how they would make decisions about their health care including DNR and healthcare proxy
• Meet needs of residents who are grieving/coping with loss
• Find options which meet resident’s physical and emotional needs Conduct intitial social service assessment (social history), interview resident and family to establish needs and problems 
• Conduct an ongoing assessment of social service plan and revise accordingly
• Record interim progress notes and quarterly evaluations
• Participate and attend interdisciplinary care plan meeting for assigned residents
