Warren Haven, in conveniently located Oxford (Warren County) NJ, is seeking a part-time Social Worker to join our team. The Social Worker will provide supportive and concrete services to our residents and their families, participating in resident assessment, care planning, MDS completion and discharge planning. Knowledge of community resources would be a plus. The individual must have excellent communication and organizational skills as well as the ability to work independently as well as in collaboration with the other team members. Previous LTC experience necessary. LSW or CSW required.
