Atlanta Regional Commission is Hiring a Community Services Coordinator Near Albuquerque, NM

Under the direction of the Community Services Department Manager, this position will coordinate services to access and participate in activities and functions of community life for individuals served. Works as part of a team to ensure person centered services. Reports on the progress of individuals receiving services, verbally and in writing to appropriate sources and will work with interdisciplinary team members in the development of the Individual Services Plan (ISP) and required documentation. Will assist with routine contact at community and home sites to ensure quality of service. Provides supports to individuals employed and participating in integrated settings throughout the Albuquerque area. Ensures Community Specialist is trained and familiar with each individual on their caseload to access their community as the individual desires. Must be able to communicate effectively with individual supports diverse individual's teams and groups. Will be required to carry out in a variety of settings. Will be responsible for heavy lifting and long periods of standing; physical demands vary greatly from community setting. Driving to various locations in an insured, enclosed vehicle is required. Must be certified in Mandt, AWMD, CPR and have the ability to work with individuals with challenging behaviors (paid training provided). Works in busy, fast-paced environment and a variety of work settings indoor and/or outdoors in all kinds of weather conditions. Must have reliable transportation and the ability to transport at any time. Will be required to operate large passenger vehicles including those with wheelchair lifts. Transportation supports may be substantial. Must be able to handle a diverse caseload, frequent interruptions and matters that require immediate attention. Time management skills are essential. Must be able to prioritize tasks and teach others how to do so. Ability to escort people on community activities in all types of weather. *** Must be able to usepersonal vehicle for daily transport***
High School diploma or GED is required. Experience working with individuals with behavior challenges preferred. Experience working with persons with developmental disabilities individually and in groups, for at least six months, is preferred. Experience with community outreach programs preferred. Knowledge of Day Habilitation practices, Community Inclusion regulations, training strategies, and supports. Must have the ability to read and carry out the requirements in an Individual Specific Plan (ISP) and adjunctive therapy plans. Ability to work with and provide effective teaching strategies to people with developmental disabilities and the staff working with them. Ability to communicate verbally and in writing in a clear, effective manner. Skill in preparing and maintaining records, writing reports and responding to correspondence. Skill in developing and maintaining effective working relationships with co-workers, employers, other organizations and the public. Computer/technology: Basic computer skills and experience using Internet/Intranet, Microsoft Windows, Outlook, Word, and Excel is strongly preferred. Ability to work with and provide effective facilitation and supports to persons with intellectual and developmental disabilities individually or in groups. Ability to escort people on community activities in all types of weather. Ability to assist with development of written goals and objectives based upon a person's vision, meaningful day desires and needs Skills in time management, scheduling and organization. Literacy staff must be able to write and implement daily lesson plans organized by the Literacy Project Manager. Ability to accurately present program to groups of people, employers, family members, residential staff and other concerned parties. Must be an ARCA approved driver and may require the frequent use of an enclosed insured vehicle. As the program continues to grow, we ask that you are flexible on changing your schedule to accommodate the needs of the individuals we serve (Evenings & Weekends)
