JOB DEFINITION
The Care Manager functions as a member of an interdisciplinary team to provide care coordination to children and their families who are diagnosed with a mental illness and/or co-occurring substance abuse disorders. The Care Manager will enhance the benefits of therapy and/or peer engagement to help sustain long-term recovery. The Care Manager will focus each trauma-facing child and family's basic needs and ensure they are being met. The Care Manger will reduce barriers and improve access to integrated care and to community-wide services addressing social/physical determinants of health (nutrition, shelter, public benefits, transport, etc.). The Care Managers role will be key to impacting our target measures such as social connectedness, housing stability, education / employment, and criminal justice system status
EDUCATION REQUIRED:
Bachelor's Degree or Master's Degree in one of the following fields preferred: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy. Degrees in other related areas may be considered.
