Family and Children's Association is Hiring a Care Coordinator Near Hicksville, NY

The Opportunity

The Care Coordinator will provide case management, health education, and referrals and follow-up for individuals receiving services in the Family Treatment and Recovery Centers. The Care Coordinator will act as the Health Coordinator of the multi-disciplinary team to educate staff and clients.

Responsibilities

  • Complete care management assessments and monitor client progress towards improving management of co-occurring physical health, mental health, and substance use issues
  • Assure the provision of education, risk reduction, counseling and referral services to all clients regarding HIV/AIDS, tuberculosis, hepatitis, sexually transmitted diseases, and other communicable diseases.
  • Administer Rapid HIV and conduct pre and post test counseling; provide referrals when indicated.
  • Refer to outside providers for Hepatitis C and B testing.
  • Monitor service plans and update, in conjunction with clinician, to resolve barriers to service utilization.
  • Ensure entitlements, insurance, and benefits are in place.
  • Coordinate with primary care and mental health providers, as well as on-site clinical team.
  • Provide education & coaching for the client and their support system regarding medications, health behavior changes, and accessing community resources & services.
  • Enter clinical data into electronic systems accurately & efficiently.
  • Assist Medical Providers in the coordination of MAT services.
  • Coordinate and monitor transportation assistance for clients.
  • Conduct client and community outreach as needed.

Qualifications

High School Diploma or equivalent.

Six months of related experience.

Excellent verbal and written computer skills.

Proficient computer skills with Microsoft Office.

Valid New York State Driver's License.

Additional Information

Schedule: Monday-Friday, weekends as needed.

Location: Hicksville

Salary: $22-23/hr
