ORGANIZATIONAL/PROGRAM BACKGROUND:

Central City Neighborhood Partners (CCNP) was established in 1994 and incorporated as a formal non-profit collaborative in 2000 to address the socio-economic conditions impacting low-income families living in the Central City, Westlake and Pico-Union communities. With the mission of catalyzing partnerships to systemically address the multiple challenges facing low-income families, CCNP works with its partners to help families create pathways to financial security and economic opportunity through our FamilySource Center (FSC) program, financial asset-building programs, and other signature programs.

Position Overview

The Lead Prevention Navigator is responsible for the facilitation, coordination, and implementation of CCNP’s Pueblo Unido Project. The Lead Prevention Navigator will ensure that eligible youth are recruited to this program by implementing the Seeking Safety Curriculum, which focuses on preventing substance use and understanding safe sex practices. The Lead Prevention Navigator will maintain up-to-date data and coordinate referrals to ensure participants' overall wellbeing. This position will also provide program and administrative support to CCNP partners and staff to ensure the program is implemented effectively, efficiently, and consistently in accordance with grant requirements and the mission of CCNP.

The CCNP Pueblo Unido Project (PUP) Lead Navigator: will perform a broad range of duties spanning the full employment cycle, including, but not limited to providing expert feedback, support and/or monitoring of the following:

  • Conduct targeted outreach and engagement for the Pueblo Unido Project and serve as a liaison to the community
  • Facilitate group and individual sessions of youth, ages 13-24, and implement the Seeking Safety curriculum
  • Ensure participant confidentiality including written and verbal information
  • Participate in coalitions that align with the project, including prevention of substance use and HIV
  • Coordinate with the Project Director and other individuals to ensure success in project implementation
  • Conduct ongoing participant engagement, to include but not limited to resource linkage, baseline, exit, and follow-up questionnaires
  • Coordinate ongoing PUP participant services with CCNP Youth Programs in collaboration with the CCNP Youth Services Manager/Coordinator
  • Coordinate referrals to other CCNP programs, case management, or other City of Los Angeles programs
  • Participate in ongoing training to increase awareness and skills around mental health, substance use and HIV prevention
  • Maintain up-to-date data entry and other project data
  • Generate reports, file completed work in designated locations and perform ongoing quality review
  • Create social media campaigns that support the message and goals of the PUP program
  • Will collaborate with partners to extend the reach of resources and the overall success of the program
  • Attend coalition meetings and coordinate testing opportunities for youth
  • Other responsibilities as assigned

QUALIFICATIONS AND EXPERIENCE REQUIREMENTS

  • Ability to work independently as well as collaboratively with limited supervision, with diverse groups including immigrants, coalitions, public officials, and media;
  • Ability to manage databases and reporting systems in order to produce a variety of written reports
  • Excellent interpersonal, verbal and written skills required;
  • The ability to read, write and speak in English and Spanish is required, given that many of the targeted families are monolingual in Spanish. Cultural sensitivity is required.
  • Operational knowledge of the Internet and web-related technologies; Microsoft Office (Word, Excel, and PowerPoint); Windows Operating System, Zoom,
  • Ability to present in front of large groups
  • Knowledge of social media platforms, particularly Facebook, Instagram, and Twitter

EDUCATION AND CERTIFICATION REQUIREMENTS

Bachelor's degree from accredited college/university and/or 2 years as a social services case manager or group facilitator.

Master’s degree in the social work or related field preferred.

  • Ability to be resourceful, organized, and demonstrate strong follow-through
  • Operational knowledge of the internet and web-related technologies; Microsoft Office (Word, Excel, and PowerPoint); Windows Operating System.
  • Ability to manage databases and reporting systems in order to produce a variety of written reports
  • Must have a valid California Driver’s License, liability insurance and access to an automobile in good working conditions;

COVID-19 Vaccine required for all employeesCentral City Neighborhood Partners has implemented a mandatory vaccination policy effective October 1, 2021 requiring COVID-19 vaccinations for all employees (with approved religious and medical accommodations). At present, an individual is deemed “fully vaccinated” when that person has received, at least 14 days prior, either the second dose in a two-dose COVID-19 vaccine series (e.g. Pfizer or Moderna) or a single-dose COVID-19 vaccine (e.g. Johnson & Johnson). As a condition of employment, employees must submit official COVID-19 vaccine documentation.

  • Approximately 50% of time is spent sitting, while frequently required to walk, stand and bend.
  • Must be able to speak and hear well.
  • Good vision is imperative.

  • Employee will be required to lift and/or move up to 25 pounds without assistance
