Job Description: Summary of Job: This position will provide Critical Time Intervention (CTI), an evidence based model of case management to a caseload of individuals and families who have substance use or co-occurring disorders and who are experiencing homelessness. The Community Support Specialist will assist participants with locating permanent housing and will support the individual’s recovery by providing linkage and referrals to treatment and other support systems. The Community Support Specialist will also enhance participants independent living skills by empowering, providing direct support, and helping the individual/family with accessing necessary community resources. Essential Duties and Responsibilities: 1. Building a relationship of trust and care with very low-income individuals/ families who have substance use disorders; 2. Providing case management duties, including: Outreach to homeless individuals/families and to agencies who serve the homeless; Providing Housing Navigation and placement services to particpants; Conducting assessments and developing, implementing, and monitoring case plans in coordination with the assessment results, recommendations of the multidisciplinary team, and participant preferences. Establishing linkages with appropriate agencies and service providers in the area/community; Providing referrals and coordinating with other service providers to provide health care services and daily living services to participants; Educating participants on issues, such as services and resource availability and participant rights. 3. Providing direct assistance to participants in obtaining and coordinating public benefits; 4. Providing/referring for services to participants, including personal financial planning services, transportation services, income support services, fiduciary and representative payee services, legal services, child care, and housing counseling; 5. Facilitating the Critical Time Intervention (CTI) evidence-based model to program participants; 6. Completing required documentation (including progress notes) within 48 hours of contact, and entering data into the Nashville Davidson County Homeless Management Information System (HMIS); 7. Participating in community meetings focused on finding solutions for Nashville-Davidson County’s homeless population; 8. Demonstrating good clinical judgment in decision making regarding participants; and 9. Demonstrating ability to relate to participants and their families in a culturally appropriate manner.
