Center Point Healthcare and Rehabilitation is seeking a Part-Time Social Services Assistant to join its team!
Monday - Friday 9am - 3pm
The Social Services Assistant will provide essential support to the social workers, assisting them with their caseloads.
Qualifications:
- High School diploma or GED
- Previous experience in a skilled nursing or long-term care facility preferred
- Strong verbal and written communication skills
- Ability to multi-task
Job Duties:
- Complete monthly transfer logs
- Act as a liaison between providers.
- Completing and maintaining 142s
- Coordinating Discharges
- Assisting social workers with their daily tasks.
- Assessing residents and determining the type of help they need.
- Working with social worker to determine treatment plans.
- Helping and providing assistance to residents and their families.
- Researching and coordinating community services.
- Helping residents to complete the necessary paperwork to apply for assistance.
- Undertaking regular check-ins with residents.
- Documenting resident behavior, filing assessments, and maintaining rosters of relevant agencies.
