CCSCC is Hiring a Program Manager, Health Access Services Near San Jose, CA

POSITION SUMMARY:

The Program Manager for Health Access Services collaborates directly with Catholic Charities programs and healthcare partners to facilitate access to healthcare services, agency resources, and community support for low-income families in Santa Clara County within their local neighborhoods. In this role, responsibilities include supervising the health access program, managing the logistics of the mobile medical unit, overseeing support staff and community volunteers, and coordinating agency services at community outreach events. The position encompasses budget management, evaluation, staff hiring, training, and supervision, as well as volunteer management, program development, implementation, and oversight. Additionally, the Program Manager plays a key role in collaborative and partnership development, community outreach and engagement, and reporting on program status.

The Program Manager will work with all agency programs within an integrated model to provide a range of services to families and individuals, including benefits enrollment (CalWORKS, MediCal, Calfresh, etc.), drop-in mental health services, immigration legal services, TaxEZ free tax preparation, and food and housing resources. As a new role within the agency, the Program Manager is entrusted with exercising independent judgment and discretion in developing and executing the health access program. The position requires strong organizational and communication skills, along with the ability to navigate diverse communities and healthcare landscapes. Adaptability and problem-solving skills are essential for addressing unexpected challenges that may arise in ensuring community access to healthcare services.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Develop and implement a comprehensive strategic plan for the mobile medical unit.
  • Collaborate with key stakeholders to identify target communities and health access needs in those communities.
  • Oversee day-to-day operations of the mobile medical unit, ensuring efficient and effective delivery of health access services and community engagement.
  • Manage staff schedules, workflow, and resources to optimize service delivery.
  • Monitor and evaluate program performance, implementing improvements as needed.
  • Build and maintain partnerships with community organizations, healthcare providers, and local authorities to deliver health access services and community resources to neighborhood locations.
  • Represent the organization at community events and meetings.
  • Develop and oversee outreach programs to raise awareness of the mobile medical unit services within target communities.
  • Develop and manage the program budget, ensuring fiscal responsibility and resource optimization.
  • Optimize resource allocation for efficient program operation including staffing and supplies.
  • Ensure health access and community services provided meet high-quality standards.
  • Manage various grants and contracts, and ensure programmatic and fiscal compliance.
  • Ensure services are appropriately funded and work closely with the fund-development team to plan for service sustainability and strategic growth.
  • Implement and monitor quality assurance measures to ensure the delivery of high-quality healthcare services.
  • Collaborate with healthcare professionals to establish and maintain clinical protocols.
  • Collaborate with community leaders to address healthcare needs and concerns.
  • Lead, coordinate, and promote interagency collaboration and interface amongst community agencies and leverage programs, services, activities and resources.
  • Recruit, train and supervise a team of support staff and community volunteers.
  • Facilitate training programs and professional development opportunities for staff and volunteers.
  • Conduct regular performance evaluations and provide constructive feedback.
  • Collect and analyze data on program outcomes, patient demographics, and community impact.
  • Prepare regular reports for stakeholders, highlighting achievements and areas for improvement.

QUALIFICATIONS:

EDUCATION AND EXPERIENCE

  • Minimum of a Bachelor's degree in Healthcare Administration, Public Health, or a related field; a Master's degree highly preferred.
  • Proven experience in program management, preferably in public health or healthcare services.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

  • Bilingual/ biliterate in English/Spanish or English/Vietnamese preferred.
  • Strong leadership and organizational skills with the ability to effectively manage a multidisciplinary team.
  • Knowledge of healthcare regulations and quality assurance processes.
  • Excellent communication and interpersonal skills for building and maintaining relationships with diverse stakeholders.

PHYSICAL REQUIREMENTS

  • Ability to lift 25 pounds
  • Ability to drive between agency sites; local travel within Santa Clara County

OTHER QUALIFICATIONS:

  • Criminal background check via livescan fingerprint
  • Must have TB test performed and submit results
  • Provide proof of COVID-19 vaccination and booster status
  • Automobile, valid driver’s license and auto insurance per agency policy; or have access to reliable transportation

WORKING CONDITIONS: 

  • Work may be performed at multiple sites and may be physically demanding.

HOURS AND OTHER CONDITIONS: 

  • Standard office hours are Monday through Friday, from 8:00 a.m. to 5:00 p.m.
  • This is a full-time, exempt position. Some evenings and weekends may be required to meet the needs of the program.
