CCC Brand is Hiring a Clinical Supervisor Near Portland, OR

The Integrated Care Team at Old Town Recovery Center is a program that seeks to support the people we serve by coordinating care across a variety of supports. The majority of people enrolled in our services have significant mental health needs as well as physical health needs that they are navigating in the community. The team includes Peer Support Specialists, Case Managers, Employment Specialists, Housing Specialists, PMHNPs, and Counselors/Social Workers who work closely together to provide individualized support to navigate core areas of need for the people we serve. The team provides onsite counseling for clients as well as outreach support in specific positions. We also coordinate with Old Town Clinic primary care providers who work out of the same building to navigate the intersection of physical and mental health needs

The Integrated Outpatient Clinical Supervisor provides clinical supervision and support to the Integrated Outpatient team at the Old Town Recovery Center. This includes direct clinical supervision to the mental health counselors, case managers, behavioral health assistants, peer support specialists, and employment specialists. This classification can provide clinical supervision to QMHA, QMHP, and PSS/CRM credentials and can provide OAR supervision to licensed staff.

The Integrated Care Teams provide mental health counseling and case management services to adults with severe and persistent symptoms of mental illness and substance abuse. The teams work to address all treatment needs, including crisis management, substance abuse treatment, medication management, physical health treatment, mental health treatment, accessing benefits and entitlements, accessing appropriate and affordable housing. The teams serve as a primary source of care coordination that is person centered, population driven, data driven and evidence based to ensure the full range of client needs is met. This position has access to mental health billing and service records.

Schedule: Monday-Friday 8:30-5:00pm 

Location: Old Town Recovery Center in Portland on the corner of Broadway & Burnside

MINIMUM QUALIFICATIONS

  • Must be registered as a Clinical Social Worker Associate (CSWA); OR Licensed Professional Counselor Associate (LPCA); OR Marriage and Family Therapy Associate (MFTA).
  • Requires 2 years of post-graduate experience in mental health. 
  • Requires 1 year of lead or supervisory responsibilities required OR demonstrates leadership abilities from an internal directly relevant role at CCC.
  • Must possess a current driver’s license.
  • LPC/LCSW/CADC preferred. 
  • Must have current CPR certification prior to start date.
  • Must demonstrate competence in leadership, wellness, oversight and evaluation of services, staff development, individual service and support planning, case management and coordination, utilization of community resources, group, family and individual therapy or counseling, documentation and rationale for services to promote intended outcomes and implementation of all provider policies. 
  • Has the competencies necessary to:
    • Identify precipitating events
    • Gather histories of mental, emotional land physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts; 
    • Assess family, social and work relationships 
    • Conduct a mental status assessment 
    • Document DSM diagnosis 
    • Write and implement or supervise implementation of a care plan 
    • Conduct and document a mental health assessment 
    • Provide mental health treatment and rehabilitative services within ones scope of practice 
  • Must meet CCC privileging requirements as required by FTCA. 
  • Must pass a pre-employment drug screen, TB Test, and criminal background check. This includes clearance by the DHS Background Check Unit. 
  • The OAR prohibits SUDS program staff from having within the previous 3 years a DUI conviction or diversion, a felony conviction, or a conviction under the Uniform Controlled Substances Act. 
  • Ability to adhere to Central City Concern’s drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Work Place Act of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol. 
  • Must possess a current driver’s license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern’s Fleet Safety policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training. Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage. 
  • Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern. 
  • Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects. 
  • Must be able to climb stairs several times a day. 
  • Must adhere to agency’s non-discrimination policies. 
  • Ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, political affiliation, cultural backgrounds, life-styles and sexual orientations, and treat individuals with respect and dignity. 

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

  • Provide and document at least 2 (two) hours of supervision per month for each full-time staff supervised. The two hours must include one hour of face-to-face contact for each person supervised, or a proportional level of supervision for part-time staff providing clinical or other support services to clients. Supervision may take place in the community during clinical interactions. 
  • Monitor ORS/SRS data in OpenFit and incorporate feedback into regular clinical supervision in order to support client-clinician therapeutic alliance associated with positive treatment outcomes. 
  • Collect information regarding clients’ history through a combination of interviewing, records review, and consultation, after obtaining written consent. 
  • Conduct and document Mental Health Assessments, including mental status exam, DSM diagnosis, and clinical formulation. 
  • Develop care plans with client. 
  • Participate in clinical staff meetings and case conferences with team members in order to utilize interdisciplinary expertise and facilitate individual treatment plans for clients. 
  • Share addictions expertise by working collaboratively with the multidisciplinary treatment team, modeling integrated treatment skills and training other staff in evidence-based principles and practice. 
  • Support the Program manager in ensuring that daily activities are being monitored, scheduled and coordinated with the team. 
  • Assist with critical incident reports and critical incident debriefings. 
  • Oversee appropriate management and maintenance of patient records system. 
  • Participate in setting goals and objectives for program, facilitating the achievement of those goals, evaluating progress, and modifying processes and/or objectives as needed based on objective data. 
  • Collaborate with Residential facilities, Adult Foster homes or other housing providers to provide necessary treatment services and coordinate residential and non-residential treatment and service planning as needed. 
  • Participate in admission and discharge planning for clients needing hospitalization or subacute level of care. Client contact must be made within one working day of admission. 
  • Monitor and assure that the health and safety needs of clients are met. 
  • Assist Program Manager in processing client grievances as needed and indicated. 
  • Participate in Outreach activities as necessary to offer services and engage participation in a non-coercive manner. Escort clients to meetings and appointments as necessary. 
  • Maintain client records, using an electronic medical record, as required by program standards and funding requirements including documentation of clinical interactions, behavioral health assessments and individual service plans. 
  • Attend required meetings and CCC trainings in a timely manner. 
  • Support Program Manager in ensuring that clinical privileging requirements are met for all direct reports pursuant to CCC’s Privileging Policy. 
  • Participate in evaluating clients for risk of harm and initiate Directors Custody transport holds when appropriate. 
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee. 

DESCRIPTION OF OTHER DUTIES: 

  • Identify and coordinate staff training needs. 
  • Coordinate services with all members of client’s support network as appropriate. 
  • Provide in-service trainings on clinical requirements; assessments and treatment. 
  • Observe mental health and addictions groups and provide feedback on facilitation to group leaders as appropriate. 
  • If licensed provide licensure supervision to staff pursuant to the standards of the individual licensure boards. 
  • Work with program manager to expand and maximize service delivery. 
  • Conduct educational and therapeutic groups for clients, using approved curriculum. 
  • Liaison with partnering agencies and programs. 
  • Perform other duties as assigned.

  • Sufficient manual dexterity and physical ability to perform assigned tasks. 
  • Knowledge of de-escalation methods or ability to be trained in de-escalation methods.
