Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

We currently have an opening for a Lead Navigator for our SSVF program!

A Lead Navigator's primary role is working with individuals dealing with chronic homelessness as their Navigator for housing, services and ongoing support. The Lead Navigator will be responsible for coordinating priority issues on the schedule and delegating tasks appropriately, updating Navigator caseloads, reviewing and submitting new client referrals to the Associate Director of Housing and Community Support, and ensuring all program spreadsheets and reports are up to date. The Lead Navigator serves as the bridge for the patient, in coordinating and monitoring RHBA and Non-RBHA community based services and facilitates delivery of these services by supporting the individual. The Lead Navigator will work with individuals who have been determined as the most vulnerable homeless according to the vulnerability index and will be responsible for conducting outreach, engagement, case management, and ongoing wrap around supportive services. Lead Navigator will also map out the most appropriate next steps for the individual who may need assistance in accessing various services after or before the individual has been housed. The Lead Navigator works in conjunction as part of a multi-disciplinary team made up of other Community Agencies and stakeholders. Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Polices and Procedures, and specific for the Bonus Project. The Lead Navigator will also carry out other duties assigned by immediate supervisor and/or clinical supervisor. A Lead Navigator will document and track required patient interactions according to the Arizona Administrative Code (AAC),

Skills and Requirements:

  • Credentialed Behavioral Health Technician (CBHT), who has one of the following:
    • Master's degree or bachelor's degree in a field related to behavioral health,
    • A registered nurse or a physician assistant who is not working as a medical practitioner
    • Bachelor's degree and at least one year of full-time behavioral health work experience
    • Associate's degree and at least two years of full-time behavioral health work experience
    • High school diploma or high school equivalency diploma and with a minimum of 2 years of mixed behavioral and physical health work experience and/or 2 years of substance abuse/mental health recovery-oriented service. Recovery oriented service experience will be verified whenever possible however respect must be paid to confidential aspects of different community resources who are based in anonymity. Must also have completed 160 hours of behavioral health, substance abuse or physical health training post high school education or via continuing education units through on-line learning management systems, college courses or conferences and seminars.
  • Valid AZ driver’s license with a 39 month clean MVR
  • HS diploma or a verified general equivalency diploma (GED)
  • Minimum of 2 years of mixed behavioral and physical health work experience
    and/or
  • 2 years of substance abuse/mental health recovery oriented service.
  • The Lead Navigator is an individual who is in recovery from substance use and/or General Mental Health for an appropriate period of time (greater than or equal to one year)
