Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. Community Bridges, Inc. is the largest non-profit provider of substance abuse/behavioral health treatment in Arizona.

We currently seek a Peer Support Specialist (PSS) for Continuing Care Services for our Phoenix, East Valley Detox location.

The Peer Support Specialist will assist in the transition to the most appropriate level of care available in conjunction with the established treatment team. This includes ensuring the coordination of care with all those parties who are currently involved in the patient’s case. The Peer Support Specialist works in conjunction as part of a multi-disciplinary team, and is responsible for the coordination of care, transition and continuing care planning, individual and group support, discharge planning, and outreach and follow-up.

Minimum Requirements:

• The Peer Support Specialist is an individual who is in recovery from substance use for an appropriate period of time (greater or equal to 12 consecutive months).

• Must be a minimum of 21 year of age to work at a CBI Facility.
• HS Diploma or equivalent.

• Six weeks of behavioral health work experience or six weeks of continuous onsite direction from a behavioral health professional, a behavioral health technician, or a behavioral health paraprofessional.
• AZ Driver's license with 39 month clear MVR
