Community Bridges, Inc. is the largest non-profit provider of substance abuse and behavioral health treatment in Arizona. We currently seek a Navigator II for our VP4 – Victory Place program at our Jones Ave location.

The VP4 – Victory Place program provides rapid rehousing and homelessness prevention for veterans through a rapid rehousing intervention in an effort to quickly help the veteran access permanent housing following the SSVF Core Concepts designed to work toward ending veteran homelessness.

Navigators identify, engage, and establish relationships with Veterans while they are living on the streets and continue to provide support after they transition into housing. The Navigators work with Veterans to link them to whatever is needed to end their homelessness and sustain their housing stability.

The Navigator will also map out the most appropriate next steps for the individual who may need assistance in accessing various services after or before the individual has been housed. The Navigator works in conjunction as part of a multi-disciplinary team made up of other Community Agencies and stakeholders, and CBI Peer Support Services & Outreach Management Team. The Navigator will document and track required patient interactions according to federal, state, local and CBI policies.

Minimum Requirements:

• Master's degree or Bachelor's degree in a field related to behavioral health is preferred.
• Registered Nurse or a Physician Assistant who is not working as a medical practitioner, and has a bachelor's degree and at least one year of full-time behavioral health work experience; or has an associate's degree and at least two years of full-time behavioral health work experience,

OR

• HS Diploma or equivalentwith a minimumof 2 yearsof mixed behavioral and physical health work experience and/or 2 years of substance abuse/mental health recovery oriented service.
• Recovery oriented service experience will be verified whenever possible however respect must be paid to confidential aspects of different community resources that are based in anonymity.
• Must also have completed160 hoursof behavioral health, substance abuse or physical health training post high school education or via continuing education units through on-line learning management systems, college courses or conferences and seminars.
• Requires a valid Arizona Driver’s License and 39 month clear MVR
