About the Program and Position:
Under the supervision of the Assistant Vice President of the Assigned Region, The Assistant Director of Care Management Services oversees the activities of agency Care Managers to ensure the provision of high-quality, family-driven, youth-guided service that is in accord with the policies and regulations of the agency, the Health Home and NYS Department of Health. The Assistant Director will support Care Managers in their efforts to help families and children advance the health care status of youth eligible for Health Home service.
Our program advocates for families and children, guiding them through complex systems and helping them access vital resources such as healthcare, education, and social services. We serve by providing comprehensive support and coordination of services to address specific needs and challenges faced by families. And this MAKES A DIFFERENCE by promoting stability, well-being, and overall development of children and families.
Health Home Care Management helps families manage the health and well-being of children and youth aged 0-21 who are enrolled in Medicaid. The program promotes the health of children and families while reducing emergency interventions, truancy, and hospital care. This makes a difference by ensuring that children receive the care they need to be healthy and happy, and families can feel confident in managing their child’s health. And the role you play in all this is to oversee the activities of agency Care Managers to assure the provision of a high-quality, family-driven, youth-guided service that is in accord with the policies and regulations of the agency, the Health Home and NYS Department of Health. The Assistant Director will support Care Managers in their efforts to help families and children advance the healthcare status of youth eligible for the Health Home service.
How you get to use your skills…
Provide consultation to Care Managers concerning outreach, enrollment, the development of Plans of Care and the conduct to Care Managers continuing care management activity
Observe agency personnel policies and assure compliance by supervisees
Review documentation by supervised Care Managers, confirming regulatory adequacy and timeliness
Assure that care management activity is consistent with the principles, regulations and policies associated with Health Home care management
Provide supervision to Care Managers in multiple counties
Collaborate and attend required trainings for multiple Health Homes
Utilize the analysis of Quality Improvement data
Evaluate impact of care managers’ activity on the healthcare status of clients, using Quality Improvement data and observation
Perform other duties as assigned
Physical Requirements
Driving to and from appointments and activities as assigned
Word processing: Including responding to emails, completing computerized documentation
Sitting for extended periods of time
Walking distances
Walking up and down stairs inside/outside
Able to have contact with consumers - non-contagious illness
Schedule: Exempt
This is a full-time position, Regular hours entail 9:00 am - 5:00 pm, Monday through Friday
Must be flexible to meet program needs
