- Provides comprehensive case management services to all clients including: intake assessment, goal setting, case plan development, progress monitoring, advocacy and referrals.
- Meets with clients in their home and/or community as directed by the case plan.
- Inputs accurate and complete documentation for all contacts with clients into agency database.
- Bachelor’s degree in Social Work
- LSW license is required
- One (1) year experience working with older adults, two (2) years preferred
- Knowledge of community resources
- Must have a current Ohio driver’s license
