What they are looking for:
•Graduate of an accredited college/university with a degree in social work (BSW required).
•Current LSW licensure in the state of Ohio.
•Experience in a discharge planning in a hospital setting is preferred.
•Knowledge of CMS Conditions of Participation for discharge planning is preferred
Hours and compensation potential:
•Position is full time primarily day shift(Mon-Fri 7:30am-4:00pm).
•Range is between $47K-$70K depending on experience and a full benefit package.
What you will be doing:
•Provides, coordinates and facilitates an appropriate plan in collaboration with other health care professionals during hospitalization, ED visits and/or visits to the Center for Cancer Care as necessary.
•Demonstrates critical thinking to organize and assure optimal patient transitions of care, within the restrictions of provider guidelines and/or limitations.
•Provides emotional and environmental support for patients, family members or visitors. Assists in providing resources for patients with complex needs.
•Adheres to organizational policies and procedures; regulatory/accrediting body requirements; and professional practice standards
•Identifies, evaluates and re-evaluates the patients’ physical, psychosocial and behavioral needs.
•Collaborates with the patient, family/designated caregiver and health care team to establish and coordinate appropriate plan/discharge plan.
•Works effectively obtaining referrals and prior authorization with outside agencies to ensure a safe and appropriate level / transition of care (i.e. APSI, Home Health Care, DME, SNF, Hospice, LTAC, Acute Rehab, and /or Chronic Care / Transition Care Team).
•Makes referrals to the appropriate community agencies as determined by individual situations / cases (i.e. Behavioral Health Partners, The Department of Job and Family Services, Addiction Services, Adoption Services, etc.).
