Duties and Responsibilities:

 Operations:

• Ensures identification and eligibility process for potential consumers is followed.

• Ensures case management services including rental and mortgage assistance, utilities assistance, disbursement of food vouchers and referral and referral services that are individually tailored to the needs of consumers.

• Ensures all information is entered correctly and timely into the agency’s consumer database.
• Ensures Intake, Comprehensive Assessment, Assessment Matrix, Housing Stability Plan, follow-ups, case closing, After Care and Follow-up are completed as required by regulatory body.
• Ensures the coordination of supportive services being provided directly or by referral meet the recipient’s needs.
• Ensures a home inspection is completed to ensure habitableness, as applicable.
• Ensures file documentation is maintained according to regulatory body requirements.
• Reviews financial packages before submission to Regional Director for approval.
• Reviews Census projection to ensure number of cases to be served is met.
• Serves as the liaison between the Family Enrichment Specialist and the landlords to ensure the partnership is maintained and consumers have access to appropriate housing.
• Member of the PQI team member to ensure that program maintains compliance with standards set forth by Catholic Charities, COA and funding source(s).
• Maintain a caseload, as needed.

 Administrative:

• Completion and submission of timely and accurate Incident reports as per Catholic Charities and regulatory guidelines.
• Completes and submits internal and regulatory reports within the required time frame.
• Performs monthly file reviews to ensure file documentation is complete and complies with regulatory body requirements.

Human Resource Management

• Provides supervision, direction, and support to program staff.
• Ensures orientation and training is provided to staff according to regulatory body requirements.
• Ensures that staff receives required training and professional development within the specified time frames.
• Provides on-going supervision to each staff member under their supervision.
• Conducts performance evaluations and/or improvement plans for all staff directly under his/her direct supervision.

Additional Duties:

• Communicate any problems/concerns to supervisor in a timely manner.
• Completes all required training as per regulatory bodies, funding, licensing, and other trainings as assigned by Supervisor.
• Attends mandatory conferences and trainings as scheduled for professional development.
• Complies with all policies, procedures, and requirements necessary to perform the functions of this position.
• Performs other duties as assigned by supervisor.

Physical Demands:

• The work is usually sedentary, but may require long periods of standing, walking and /or bending and some light fitting. The work is mostly performed in a secure office setting.
• Travels local and out of state if necessary.

Educational & Experiential Requirements:

• An advanced degree in social work or comparable human service field and at least two years of direct experience in service delivery; or
• a bachelor’s degree in social work or comparable human service field and four years of direct experience in service delivery.
