About the role

  • This role will be based in our Latina’s y Nino’s program aimed to serve women in recovery. Latinas y Niños is a 6- to 12-month addiction treatment program. Latinas y Niños focuses on the unique cultural and social needs of Latina women in recovery and their children. Children under the age of 10 may live with a parent or guardian who is in recovery at Latinas y Niños. Our goal is to offer families a safe, supportive environment to heal from trauma and develop their own unique pathway to recovery.
  • As part of the Casa Esperanza, Inc. team you will provide services in a fully integrated, holistic, and collaborative manner using a team-treatment approach, to ensure the coordination of all client services, both across Casa programs as well as with external providers. This integrated model aims to facilitate clear communication amongst all providers and eliminate duplication of efforts or conflicting approaches to care; detail necessary follow up and assignments; and ensure-wrap around support services are in place to maximize treatment effectiveness and clinical outcomes.


What you'll do

  • Communicates and engages with patients regularly and helps enrollee progress on goals identified in Patient Centered Treatment Plan
  • Working with the nurse and clinicians on their health and safety needs involving community-based providers
  • Maintain weekly progress notes within the electronic health record
  • Facilitate group therapy services to the patients enrolled in the program
  • Triages and troubleshoots care need issues for patients; works with clinical team internally and in partnership with Casa Esperanza’s outpatient programs
  • Participates in team case conferences and advocates for patients’ needs
  • Participates in weekly multi-disciplinary team meetings.
  • Reminds patients about keeping appointments, filling prescriptions, etc.
  • Requests & sends medical records to for care coordination purposes to providers, ACO, MCO as needed.
  • Knowledge of relevant community resources for continuing care/aftercare placement.
  • Responds promptly to client needs
  • Maintains confidentiality
  • Completes discharge notes and client disenrollment forms to close cases upon client discharge
  • Attends staff meetings and supervision sessions
  • Collaborates with patient’s external partners such as Probation, parole, DCF, DMH, etc.
  • Develop with patient an aftercare plan and goals for referrals to community resources.
  • Be flexible and able to multi-task
  • All other related duties as assigned

Qualifications

  • Associates or bachelor's degree
  • Bilingual English/Spanish, required.
  • Excellent customer service skills and the ability to communicate professionally with employees and patients both on the phone and in person
  • Efficient, organized, detail oriented, and able to complete tasks in time sensitive manner
  • Self-directed with the ability to work both independently and as part of a team and interested in using data to help direct care
  • Motivational Interviewing and Cultural Competency training a plus; experience working with homeless populations and/or populations with mental illness and substance use disorders.
  • Experience in case management or human services is preferred but not required
  • Computer skills: Proficient with Microsoft Excel Spreadsheets, entering narrative and other data into a database, the use of the Internet to conduct information searches.

Schedule:
Mon - Fri 8am to 5pm
(Evening requires based on Program needs)

While performing the duties of this job the employee is regularly required to walk, stoop, kneel, crouch or crawl and talk or hear. The employee must occasionally lift and/or move up to 20 pounds.
