Job Description:
As an In-Home Healthcare nurse, your role is pivotal in delivering high-quality patient care. You will:
  • Conduct assessments of patients' health status and aid in administering medications and treatments per the patient’s individualized care plans, developed in collaboration with our clinical healthcare team.
  • Performing nursing procedures and interventions such as wound care, and IV therapy as prescribed by the clinical nurse supervisor.
  • Providing education and support to patients and their families on managing health conditions, medications, and activities of daily living.
  • Monitoring patients' vital signs, symptoms, and overall health status, and reporting any changes or concerns to the supervising RN or physician.
  • Collaborating with other members of the healthcare team, including RNs, therapists, and aides, to ensure coordinated and comprehensive care.
  • Documenting patient care activities accurately and timely in accordance with regulatory standards and agency policies.
Qualifications:
To be considered for this role, you should meet the following requirements:
  • Active and unrestricted LPN license in the state of Kansas / Missouri.
  • At minimum, current BLS certification (Certification requirements vary by location).
  • At least 1 year of experience as an LPN, actively within the last 12 months.
  • Strong clinical and critical thinking skills.
  • Exceptional communication and interpersonal abilities.
  • Ability to work in a sensitive environment while maintaining appropriate composure.
  • Previous experience in home healthcare or a similar setting is preferred but not required.
