We are seeking to hire an experienced Registered Nurse to join our ever-growing interdisciplinary team of Home Health and Hospice professionals.

Our mission is to empower our clinicians to contribute to the health and well-being of every patient placed in our care, throughout their lifetime, by utilizing evidenced based best practices.

RN Case Management Expectations:

• Provides direct patient care in their home.
• Oversees the patients’ plan of care.
• Coordinates the patients’ ongoing skilled care under the direction of patients' physician.
• Promotes positive patient outcomes.
• Participates in patient interdisciplinary case conference.

• Annual salary ranges from $75,000-$85,000 depending on years of experience

Requirements to join the team as a RN:

• Registered Nurse license required
• Ideal candidates will have home care experience but we are open to training RNs with a ICU/Med-Surg/Emergency Room background
• Valid driver's license and auto insurance
